Motor Vehicle Report
The Motor Vehicle Report (MVR) Release form authorizes Clackamas Insurance Agency to obtain a copy of an employee’s driving record and share that information with the employer and the employer’s insurance company.
This form can be viewed and printed using the link below. It must be signed by the employee (the driver) and the employer, then faxed to the Clackamas Insurance Agency.
